It was recently recommended in our PHA revalidation that we install NH3 detectors in our relief headers. I've done some research and talked to some technicians who say that these 1-2% relief line sensors are notoriously unreliable. A consultant heard that some people are placing a small stub of pipe on the relief header. At the far end of the pipe is a pressure sensor with a 10# trip.
Sensor probes are designed to be used in vertical position (with sensor looking to the ground). In this position, the protection cap of each sensor probe is effective against bad weather conditions. Each model has six dedicated sockets to connect sensor probes. They are located in the sensor side, as shown below. Each socket has a protecting cap.

There is no reason a chip should go bad though. What has been mentioned above are all possibilities. Corrupt cards, transfer errors, or some sort of glitch in the electronics. What Joves suggested is the best approach. Keep narrowing the problem down till you find out what is causing it.
